Print this chart by clicking on this link. Make sure that you print this in A4 size sheet. Carefully choose 'print to size' and not 'scaled to fit' for correct results.

STEP 2: TIME TO TEST  

Before the test, remove your glasses. Now hold this chart in your hand and hold it straight right in front of your eyes 14 inches away. Start reading from the topmost line and keep on reading till you reach a line where the letters are not visible to you. Now that's your reading power.
